Triple H Housing, Health, and Humanity

Based on the principles of population health, which blend healthcare with social factors, the California Department of Health Care Services launched the CalAIM initiative to reform Medi-Cal and improve health outcomes, especially among the most vulnerable population segments. Working in concert with regional health plans, CalAIM provides concrete tools like Community Support Services (CSS) and Enhanced Care Management (ECM) to fill gaps in care among homeless individuals and other vulnerable groups.

In partnership with the Central California Alliance for Health (CCAH) health plan, Community Homeless Solutions' Triple H Program implements CSS and ECM services for homeless individuals and families in Monterey and San Benito Counties, including those fleeing domestic violence and those displaced by floods, fires, or other tragic events.
