Transitional and supportive housing with trauma-informed services for young adults ages 18–24 experiencing homelessness.
What is Serenity YHDP?
The Serenity Youth Housing Demonstration Program (YHDP) is dedicated to helping youth experiencing homelessness achieve stable housing and successfully transition to independent living. The program provides trauma-informed services and individualized support that promote safety, resilience, and the development of essential life and interpersonal skills needed to obtain and maintain permanent housing.
YHDP also connects participants with physical and behavioral health services, educational and vocational opportunities, and employment resources to improve their overall well-being, increase income, and foster long-term self-sufficiency.
Program Structure
A full continuum of care — up to 36 months total, from transitional housing through independent stability.
Transitional Housing
Up to 24 months
Safe, stable housing paired with case management, crisis intervention, and life-skills development while participants build toward independence.
Continued Support Services
Up to 12 months after exit
Ongoing support following the move to permanent housing — discharge planning, follow-up, and referrals to keep participants stable long-term.
